First Visit
As a new patient, your initial appointment may consist of teeth cleaning with one of our many experienced hygienists, x-rays as needed and an exam by one of our dentists. We will discuss the results of the exam with you and explain your diagnosis and treatment options, as needed.
Please assist us by providing the following information at the time of your first visit:

A photo ID and your completed patient information form.

A list of medications you are currently taking and your completed medical history form.

If you have dental insurance, bring your insurance card and a copy of your plan coverage. This will save time in processing your insurance claims and for estimating your payment portion if any.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the first visit.
Please alert the office if you have a medical condition(i.e. diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) that may be of concern prior to treatment or if you are taking any medication (i.e. heart medication, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings, such as antibiotics for pre-med.
If your previous dentist has taken recent x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office.
